When planning for a family member with special needs, such as for a disabled child, preparing a Life Care Plan (sometimes referred to as a “letter of intent”) is very important. The purpose of the Life Care Plan is to serve as a roadmap for future caregivers, to help them address the needs of the disabled child as effectively as possible. In preparing a Life Care Plan, you should put in writing all of the information you carry with you about your disabled child, such as the daily routines, medical history and care, daily living skills, behavorial issues, education background and goals, benefits your child receives, and your hopes for your child’s future.
Once the Life Care Plan is completed, it should be kept with your other important papers and updated annually.
